Attention CCP Students:  Please complete and submit the Material Acquisition Form listing  ALL required CC+ textbooks, access codes, and only online websites with fees to be paid by the TSCD prior to obtaining them from the campus bookstore.

Mrs. DiMatteo, from the THS Media Center, will contact you regarding the status of your request.* This process MUST be followed or families will be billed for the cost of the materials. You are NOT permitted to RENT materials.

NOTE:  Dual Credit Students, including those in DC College Writing, DC Spanish, and DC Financial Accounting, are not to complete this form.  This form is only to be used by students who are attending off-campus colleges.
